Electrophysiology: Ventricular tachycardia: diagnosis of broad QRS complex tachycardia.
نویسنده
چکیده
having a broad QRS complex, it is important to be able to diVerentiate between a supraventricular and a ventricular tachycardia. Medication given for the treatment of a supraventricular tachycardia (SVT) may be harmful to a patient with a ventricular tachycardia (VT). 2 A reasonable haemodynamic condition during a tachycardia may erroneously lead to the wrong diagnosis of SVT. Familiarity with the ECG signs allowing the diagnosis of a VT is therefore essential. But as will be discussed here, the ECG should not only tell you how to distinguish VT from other tachycardias with a broad QRS complex, but also to suspect its aetiology and its site of origin in the ventricle. Both aspects are important in decision making about the prognostic significance of VT and correct treatment. Classification of tachycardias with a broad QRS complex

Ventricular tachycardia as default diagnosis in broad complex tachycardia.
In the differential diagnosis of broad-complex tachycardia, the most important decision is whether or not the tachycardia is ventricular, since this type carries the worst prognosis. However, the rules for a diagnosis of ventricular tachycardia are so complex that they are not satisfied in many cases, and the default diagnosis, supraventricular tachycardia, is erroneously accepted. We sought to...
متن کاملNoninvasive diagnosis of a wide QRS complex tachycardia mechanism by programmed atrial and ventricular stimulation using a dual chamber defibrillator.
Stored atrial and ventricular electrograms retrieved from dual chamber implantable defibrillators facilitate the diagnosis of arrhythmias. This case also illustrates the usefulness of programmed atrial and ventricular stimulation for noninvasive rhythm diagnosis in patients with a wide QRS tachycardia and an implantable defibrillator.

متن کاملMultivariate analysis to simplify the differential diagnosis of broad complex tachycardia.
Univariate analysis has identified several criteria that aid the differential diagnosis of broad complex tachycardia. In this study of 102 consecutive patients multivariate analysis was performed to identify which of 15 clinical and 11 electrocardiographic variables were independent predictors of ventricular tachycardia. These were shown to be a history of myocardial infarction, the QRS wavefor...
متن کاملTermination of a narrow QRS-complex tachycardia: what is the mechanism?
A 27-year-old man with a 10-year history of recurrent episodes of paroxysmal supraventricular tachycardia underwent an electrophysiologic procedure. The baseline sinus cycle length was 860 msec, and the atrial-His (AH) and Hisventricular (HV) intervals were 80 and 45 msec, respectively. A narrow QRS-complex tachycardia was induced by atrial overdrive pacing. The cycle length of the tachycardia ...
